This form is a contract with legal consequences.
Please read it carefully before agreeing to confirm your participation in Fortem Australia activities and events.
- I intend to participate in current and/or future activities and events with Fortem Australia Limited (“Fortem Australia”). I understand and acknowledge that such activities and events may involve risk.
- I understand that the activities and events of Fortem Australia may involve conditions, situations, and occurrences that may be variable and unpredictable, that may change without notice, and that may not be able to be foreseen. I also understand that the activities and events of Fortem Australia may require physical exertion and that I should consult a physician before participating in such activity.
- Without limiting the nature of the foregoing, I understand that Fortem Australia has no control over such risks. I also understand that such risks may cause injury, loss and/or damage. In consideration of acceptance of my participation in activities and events with Fortem Australia:
- I hereby release, waive and forever discharge Fortem Australia, its officers, directors, members, volunteers, employees, agents and sponsors, executors, administrators, heirs, successors and/or assigns (“the Organisers”), from all claims, demands, costs, expenses, actions and causes of action, whether in law or in equity, in respect of death, injury, loss or damage, to my person or property, howsoever caused, arising out of, or in any way connected with my participation in any activity and event of Fortem Australia, whether as a spectator, member, participant, competitor or otherwise, and whether prior to, during, or after the same.
- I agree to indemnify and keep indemnified Fortem Australia, and the Organisers, against any damage, claim, demand, or cause of action, arising out of, or in any way connected with my participation in the activities and events of Fortem Australia.
- I agree to comply with all rules, regulations, and instructions of Fortem Australia and the Organisers.
- In the event of injury or illness during participation in a Fortem Australia event or activity, I consent to receive medical treatment, as deemed necessary or advisable by Fortem Australia and/or the Organisers.
- I acknowledge that I have a responsibility for my personal safety, and the safety of my possessions and equipment during all events and activities with Fortem Australia.
- Unless I advise otherwise, I hereby permit the free use of my image in any broadcasts, telecasts, and in the press, including website and social media, as they pertain to Fortem Australia and its events and activities.
